To apply for a Dual Eligible Special Needs Plan (D-SNP), you must qualify for both Medicare and Medicaid. You need to meet the federal government’s requirements for Medicare eligibility, and your state’s specific requirements for Medicaid Eligibility. To qualify for Medicare, you likely need to be:

Age 65 or older, or;
Under 65 with a qualifying disability, or;
Diagnosed with end-stage renal disease (ESRD) or amyotrophic lateral sclerosis (ALS, also called Lou Gehrig's diesease).

If you are dual eligible, you do not need to wait until the Annual Enrollment Period (AEP) to choose a plan.
